By submitting the Offer, you accept to receive notification of the outcome of the selection procedure by electronic means. Tender for the procurement of medical equipment is organized within the "Support to the health operators in piloting telemedicine and tackle COVID- 19 spread in Moldova" Project ref no 305.2368-20, implemented by Homecare Association with financial support of Central European Initiative (CEI).

CERERE DE  OFERTĂ COMERCIALĂ

PENTRU  PROCURAREA  ECHIPAMENTULUI  ELECTROCASNIC

Asociația Obștească „HOMECARE”,  solicită oferte de preț pentru procurarea echipamentului electrocasnic specificat mai jos, în cadrul  Proiectului „Îmbunătățirea serviciilor medico-sociale pentru persoanele cu nevoi de îngrijire de lungă durată pe ambele părți ale Nistrului”  finanțat de Uniunea Europeană în cadrul Programului General UE de Măsuri pentru Consolidarea Încrederii (V) - Modernizarea sectorului sănătății și implementat de către Agenția Germaniei pentru Cooperare Internațională (GIZ) în parteneriat cu Agenția Cehă pentru Dezvoltare (CzDA).

  • Dosarul de aplicare va include:
  1. Oferta comercială cu  prețul unitar cu 0 % TVA și cu TVA, caracteristicile tehnice ale echipamentului propus, termenul de livrare, termenul de garanție;
  2. Oferta comercială va fi valabilă  timp de 30 zile calendaristice de la data limita de depunere a ofertelor;
  3. Prezentarea experienței ofertantului: prezenţa pe piaţă, clienţi fideli/referinţe;
  4. Certificatul de înregistrare al companiei;
  5. Numele persoanei de contact și datele de contact ale ofertantului.
  • Aplicarea dosarelor:
  1. Ofertele  pot fi aplicate în biroul central al AO.HOMECARE,  la adresa mun.Chișinău, Bd. Ștefan cel Mare și Sfânt nr. 119, ap.26,  până la data de 21.07.2020, ora 16.00.
  2. Dosarele vor fi vizate de semnătura persoanei autorizate și ștampilă.

ACQUISITION NOTICE

 

Subject: Acquisition of the "TELEMEDICINE PLATFORM".

The present procedure for awarding the supply contract is carried out within the project "Cross-border access to healthcare through telemedicine and modern equipment" (project code 2SOFT / 4.1 / 89), on the basis of the grant contract no. 20356 from 21.02.2020.

This Project is implemented by Homecare Association (Republic of Moldova) in partnership with Social Link Association (Romania) with financial support of European Union through European Neighborhood Instrument (ENI) within Joint Operational Programme Romania-Republic of Moldova 2014-2020.

Social Link Association, based in Romania, Iași Municipality, sat. Valea Adâncă, Miroslava Commune, str. Veche, no. 162, launches the procurement procedure by tender for the “Telemedicine Platform”. Maximum value of the contract: 20,000 Euro

The tenders response must include the following documents:

1. Technical proposal in accordance with the specifications set out in Annex 1 (Technical specifications) to this Procurement File. It is mandatory to comply with all minimum requirements imposed by these documents.

The offer will be made in full, for all requested items and in the requested configuration. Partial bidding is not allowed and no alternative offers are allowed. Offers that do not include all the requested products / services will be disqualified. Tenderers who submit alternative offers will be disqualified.

2. The financial proposal, which will be presented in accordance with Form no. 7 and Annex Price summary.The financial offer will be expressed in euro.

3. The following Forms:

- Form no. 1 - Declaration on own responsibility regarding the conflict of interests.

- Form no. 2 - Association agreement to participate in the procurement procedure - if applicable.

- Form no. 3 - Statement regarding the part / parts of the contract that are fulfilled by the associates and their specialization - if applicable.

- Form no. 4 - Subcontracting statement - if applicable.

- Form no. 5 - Power of attorney for the signatory of the offer - if applicable.

- Form no. 6 - Letter of submission.

- Form no. 7 - Financial proposal

- Form no. 8 - Statement on personal situation.

- Form no. 9 - General information.

- Form no.10. – Statement that the applicant is not in any of the situations referred to in art. 106(1) and 107 of Regulation (EU, Euratom) No 966/2012

The economic operator shall draw up the tender in such a way that it fully complies with the requirements set out in Annex 1 - Technical specifications. The offer that does not meet the requirements of the award documentation will be rejected.The validity period of the offer will be at least 120 days from the deadline for submission of offers.

Criteria for awarding the contract:

- Best quality/price ratio

- Delivery time

The offer will be submitted in English. Documents in other language may be submitted only with the authorized translation into English. Each page of the offer will be numbered and signed by the legal / authorized representative of the tender. If the offer is not signed by the legal representative, the power of attorney given by him to the person who will sign the offer will be attached to the offer (Form 5, in original).The offer will be accompanied by a letter of submission (Form 6).

The deadline for submitting the reply to requests for clarification from tenderers is July 15, 2020 untill 18.00.

Contracting Authority (Social Link Association) reservs the right to cancel the tender at any time.
