CRITERIILE DE ELIGIBILITATE SUNT:

-       Costul total al lucrărilor să nu depășească suma de 198 050,00 MDL

-       Posibilitatea de realizare a lucrărilor la cota 0 TVA

-       Furnizorul să poată executa toate materialele descrise în tabelul de mai sus

-       Dovadă că frunizorul are experiență în serviciul de tipărire de cel puțin 3 ani (se prezintă împreună cu oferta)

VALOAREA ŞI ACORDAREA CONTRACTULUI: Ofertele de preț vor fi evaluate pentru toate produsele împreună și contractul va fi acordat companiei care îndeplinește cerințele solicitate și oferă cel mai mic preț total evaluat. 

VALABILITATEA OFERTEI: Oferta dumneavoastră trebuie să fie valabilă timp de 15 zile de la data limită pentru depunerea ofertelor. 

PLATA: 25 % din valoarea produselor  achiziționate va fi efectuată în avans, 75 % din valoarea produselor  achiziționate va fi efectuată  după recepționarea produselor. 

FACTURARE: Furnizorul va emite factura în baza legislației în vigoare. 

CRITERIUL DE ATRIBUIRE: In conformitate cu cel mai mic preț.

CERINȚE față de furnizor: Furnizorul trebuie să aibă un record curat și fără datorii față de autoritățile de stat, instituțiile publice și companiile private, în caz contrar acesta va fi descalificat.

AGREMENT: prin participarea la această licitație și prin prezentarea ofertei dvs., companiile acordă agrement față informațiile solicitate pentru a primi servicii și informații despre datoriile și aspectele financiare ale companiei dvs. de la companii terțe.

 Asociaţia Obştească „Homecare”,  IDNO  1010620003467, numită în continuare „Beneficiar”, în persoana   directorului  d-nei Adaşan Tamara, care acţionează în baza    Statutului, pe de o parte,  şi 

 __________________________________, numită în continuare „Prestator”, în persoana   directorului d-/nei/nul__________________________________, care acţionează în baza    Statutului, pe de altă parte, au convenit să încheie prezentul contract, cu respectarea următoarelor clauze:

1.      Obiectul contractului

1.1. Prin contractul de prestări servicii, Prestatorul se obligă, la solicitarea Beneficiarului, să presteze serviciile, indicate în p.1.2. al prezentului contract, iar Beneficiarul se obligă să  recepţioneze lucrările şi să achite costul acestor servicii.

1.2. Prestatorul se obligă să presteze următoarele servicii:

Executarea lucrărilor tipografice în cadrul  proiectului „Support  to the Development of  Home Care services in Moldova – Southern region, 2015-2017”,  scutit de TVA, după   cum urmează: 

1.2.1.     Manual de auto-îngrijire la domiciliu  - 800 (opt sute) bucăți;

1.2.2.     Suport în îngrijirea bolnavului la domiciliu  - 1045 (una mie patruzeci și cinci) bucăți;

1.2.3.     Catalogul ONG-urilor – 30 (treizeci) bucăți;

1.2.4.     Buclete (A4 pe 2 părți)  – 450 (patru sute cincizeci) bucăți;

1.2.5.     Leflete – 1000 (una mie) bucăți;

1.2.6.     Broșuri – 400 (patru sute) bucăți;

1.2.7.     Postere – 15 (cincisprezece) bucăți;

1.2.8.     Buclete  pentru conferință – 100 (una sută) bucăți;

1.2.9.     Postere pentru conferință – 15 (cincisprezece) bucăți;

1.2.10.Bilborduri pentru conferință – 3 (trei) bucăți.

1.3.  La necesitate executarea lucrărilor tipografice pot fi efectuate pe părți aparte, conform solicitării. 

1.4.   Materialele, indicate în punctele 1.2.8 - 1.2.10 (pentru conferință), pot fi modificate în volum sau tipaj în baza anexei suplimentare la contract.

1.5.   Condiţiile lucrărilor  şi volumul lor se indică de către Beneficiar în oferta comercială,  contractul de prestări servicii și solicitări.

1.6.   În timpul executării lucrărilor  Prestatorul va prezenta  Beneficiarului în original-macheta industrială (formatul lucrării, culorile, tipul de hîrtie, produsul final) în forma Word care urmează a fi semnată de Beneficiar.

1.7. Mostrele semnate vor constitui obiectul principal al contractului pe intreg parcursul executării acestuia.

    1.8. Prestatorul poartă răspundere faţă de Beneficiar pentru calitatea, volumul și termenii de  executate a lucrărilor.

    1.9. Termenul de prestare a serviciilor – 10 zile după prezentarea materialelor necesare de către Beneficiar Prestatorului.

    1.10. Serviciile se consideră prestate după semnarea actului de predare a serviciilor Beneficiarului  sau  reprezentanţilor  lui  împuterniciţi.

 

 

2.  Drepturile şi obligaţiile Părţilor

2.1. Prestatorul este obligat să respecte termenul de prestare a Serviciilor indicate în p.1.4 al prezentului contract.

2.2.  Beneficiarul este obligat să achite serviciile la preţul indicat în p. 3 al prezentului contract în temnen de 3 zile după finisarea lucrărilor.   

2.3. Beneficiarul are dreptul în orice moment să verifice mersul şi calitatea prestării serviciilor de către Prestator, fără a se implica în activitatea acestuia.

3. Preţul contractului

3.1. Preţul prezentului contract este stabilit la 198 050 (una sute nouăzeci opt mii cincizeci) lei, TVA – 0%.

3.2. Achitarea de către Beneficiar Prestatorului se efectuează prin transferarea mijloacelor băneşti pe contul de decontare a Prestatorului, îndicat în prezentului contract.

4. Responsabilităţile Părţilor

4.1. Măsurile de răspundere a părţilor se vor aplica în conformitate cu normele legislaţiei civile, valabile pe teritoriul Republicii Moldova.

5. Ordinea soluţionării  litigiilor

5.1. Litigiile şi divergenţele, care pot apărea la executarea prezentului contract, vor fi soluţionate după posibilitate pe calea negocierilor dintre părţi.

5.2. În cazul imposibilităţii de soluţionare a litigiilor pe calea negocierilor, părţile se transmit toate litigiile spre soluţionarea în instanţa de judecată.      

       6. Clauze finale

       6.1.  Toate modificările şi completările la prezentul contract sînt valabile cu condiţia, că ele se întocmesc în scris şi se semnează de către reprezentanţi împuterniciţi ai părţilor.

       6.2. Prezentul contract este întocmit în două 2 exemplare. Fiecare  dintre părţi dispune de cîte un exemplar de contract.

Purpose of the evaluation

The final evaluation findings are expected to:

·         Evaluate achievement of specific objective and project results

·         Analyse progress, efficiency (economy), project management and organization, impact and sustainability of the project activities in relation to actual circumstances in Moldova

·         Identify deficiencies and good practices of the project – regarding the methods and tools applied

·         Provide recommendation for future actions in the field of prevention of diabetes in Moldova

Target groups

Direct:

-   Doctors from district primary care institutions: head-doctors or specialists from primary healthcare were trained to be able to act as educators for other doctors and nurses in their district.

-   Nurses from district primary healthcare institutions: selected nurses responsible for the development/maintenance of diabetes school from the primary healthcare institutions were trained to be able to act as educators for diabetic patients in their home SHCs.

-   Teachers from medical colleges: selected teachers of various subjects were trained to later apply gained knowledge in everyday work when teaching students.

-   Doctors and nurses from other medical institutions within the districts: were selected and trained by the particular District Healthcare Centre to ensure spreading of the knowledge

-   Diabetic patients: selected diabetic patients benefited of long-term regular group and individual consultation.

Indirect:

-   Persons suffering from diabetes and potential patients from all over Moldova

-   General public: benefited of information provided by the project and of free testing during World Diabetes Day

Project Background

The project is continuation and phase-out of the previous projects on diabetes prevention implemented by CCR and AOHC in Moldova during 2013 – 2015.

The project is aimed at improving accessibility of quality medical services in prevention and treatment of secondary diabetes in sake of improving quality of life of population suffering from diabetes in Moldova and reducing the death rate caused by late complications. It aimed to enhance professional capacity of medical staff from 36 medical institutions on district level in all regions of Moldova in modern diagnostics, treatment, and early detection of diabetes among high-risk population and at the same time to increase the access of the population of these districts to improved health services in secondary prevention and treatment of diabetes.

Special educational materials that were produced during previous years of the project were updated and used for training of medical staff, students of medical colleges and patients. Selected doctors from the 36 district healthcare centres were trained to deliver training on diabetes topics for other medical staff under the frame of existing life-long education for medical workers. Selected teachers from 5 Medical Colleges were trained to deliver training on diabetes topics for students. Selected nurses from the 36 district healthcare centres were trained to deliver training on diabetes topics for diabetic patients. Diabetic patients benefited from periodic consultations and testing. Awareness campaign implemented within the project informed the population about the importance of late complications’ prevention.

The overall objective of the project is Improving quality of life of population suffering from diabetes in Moldova and reducing the death rate caused by late complications.

The specific objective for the project is to increase the access to improved healthcare services in prevention and treatment of secondary diabetes for the population of Moldova.

Expected Results of the project

·         R 1 Strengthened professional capacity of medical staff from medical and educational institutions

·         R 2 The patients gain knowledge and skills to prevent complications and transform their disease into a special life style

·         R 3 Raised public awareness about the positive effects of early detection of the disease, prevention of complications and about existing medical opportunities in Chisinau

Organizational Context

  I.      Caritas Czech Republic (CCR) is a non-governmental organization with the vision of reducing human suffering, loss of lives, and poverty, enhancing the dignity of people living in poor conditions as well as building up and strengthening solidarity and participation. The traditional fields of work for CCR are social work, health care, education and livelihoods. Besides its activities in the Czech Republic, CCR, through its department of humanitarian and development aid, provides assistance abroad, in areas affected by natural disasters, war, conflicts, or poverty, both on humanitarian and development basis.

Caritas Czech Republic has been active in Moldova since 2004. It is currently implementing development cooperation projects aimed at developing a model of home care, promoting the inclusion of children in preschool education facilities, prevention of diabetes and leisure activities for children and youth. In addition, CCR has experience in implementing projects to support children abandoned as a result of migration, supporting small business, developing agriculture and actively involving civil society in decision-making processes in Moldova.

Description of deliverables

Selected supplier will conduct external final evaluation of the given project and based on the findings will draft evaluation report in English.

The evaluation report shall have between 35 and 50 standard pages (1 800 characters / page). Its structure shall be as follows:

·         Methodology applied

·         Findings (as per the structure below)

·         Conclusion and recommendation

·         Annexes (lists of interviewees and informants, questionnaires, photos etc.)

The evaluation shall focus mainly on the topics and questions below (the list is not exhaustive)

Achievement of given indicators

On the level of Results:

·         72 doctors and 108 nurses from 36 district SHCs/FDMCs and 20 teachers from 5 medical colleges should have been trained in diabetes topics and training skills (4-day trainings) – “educators”

·         At least 300 doctors ad 500 nurses from 36 districts should have been trained in diabetes topic by the educators (= by the trained 72 doctors)

·         80% of trained educators (doctors, nurses, teachers) should have improved their knowledge in diabetes topics and trainers skills

·         90% of trained educators (doctors only) should have utilised newly gained skills and knowledge in trainings for other medical staff"

·         5 groups of patients should have completed the course of regular group meetings (at least 4 meetings during 6 months, with partial supervision of expert on diabetes)

·         Patients (members of groups) should have gained new knowledge and adopted new lifestyle"

·         20 pilot lessons for students lead by trained teachers in the 5 medical colleges was held with assistance of expert on diabetes

·         Wide public should have been informed about the topic of diabetes through printed media

·         Citizens of 11 towns/villages should have been acquainted with the problems of diabetes during World Diabetes Day

·         Professional public should have been informed about project outcomes and good/bad practice during the final conference

 

On the level of Specific objective (by July 2017):

·         Increased availability of quality counselling in the field of diabetes in selected areas (the number of patients who received patient quality advice in the field of diabetes)

·         Reduced the number of patients waiting for hospitalization by 10% by 2020

REQUIREMENTS

Necessary:

·         At least 5 years of experience in evaluating development projects (verifiable list of evaluations conducted in the past required)

·         Fluency in written and spoken English, Russian and Romanian; and the ability to edit texts

·         Demonstrated excellent analytical skills (evidence required)

·         Excellent interpersonal and communication skills, and comfortable working across cultures

·         Demonstrated understanding of development sector in the field of health care (verifiable information on relevant experience required)

 

Recommended:

·         Expertise in health care sector, prevention programs, educational programs in health sector

·         Experience in working in Moldova

 

SELECTION OF SUPPLIER AND SELECTION CRITERIA

The evaluator will be selected on the basis of offers containing:

·         evaluation plan

·         description of experience and expertise (copies of documents proving the expertise and experience shall be attached)

The evaluation plan has to contain:

·         Description of the methodology to be applied

·         Timeframe

·         Budget of the evaluation (break down) and total price of the offer (the price cannot exceed the price stated below)

·         Proposed structure of the evaluation report

The evaluation plan will become an annex to the contract that will be concluded between CCR and the evaluator selected.

 

The description of experience and expertise has to include:

·         contacts to persons / institutions that can provide reference, and / or

·         copies of official documents proving the experience and expertise stated

 

The selection criteria will be as follows:

·         Offered price (50%)

·         Proven expertise and experience (25%)

·         Quality of the proposed methodology (25%)

TERMS OF ASSIGNMENT

Selected evaluator will be subcontracted by CCR to produce the deliverables specified above. The contract to be concluded between CCR and the selected evaluator constitutes Annex 1 to this call for tenders.

Maximum price to be paid for the delivery is 2 000 EUR.

The price agreed on will cover evaluator´s remuneration, all costs incurred by the evaluator related to the assignment and all taxes and fees required by law. The price will be final and cannot be increased under any circumstances.

CCR will provide the evaluator with all information and documents it has at its disposal needed to conduct the evaluation.
